The Orange Primary Access Coordination Team (OPACT) is a part of a free community program to link people without health insurance to the care they need. The OPACT team consists of a Registered Nurse, a Registered Dietician, and an Eligibility Specialist who work in the Orange County community to identify families without health insurance and:
- Help them with paperwork and conduct eligibility screenings for insurance programs such as Medicaid and Kid Care
- Conduct health screenings and risk assessments for chronic diseases
- Provide case management and health education
- Link people to a health care provider (medical home)
Our staff is multilingual and can provide services in English, Spanish and Creole.
Chronic Disease Management | >Next
The OPACT program also provides for chronic disease management for people with diabetes, heart disease, HIV and others.
The chronic disease component is managed by a Registered Nurse (RN) who works closely with a physician in one of the medical homes, a Registered Licensed Dietician and a Health Educator.
