Prior to entry, attendance, or transfer to preschools, schools (K-12), licensed childcare facilities, and family daycare homes, each child shall have on file a record of Physical Examination completed within the last 12 months and a Florida Certification of Immunization, DH680 documenting the following:
- 4 - 5 doses of Diphtheria Tetanus Pertussis (DTaP)
- 2 - 3 doses of Hepatitis B
- 3-4 doses of Polio
- 2 doses of Measles Mumps Rubella (MMR)
- 2 doses of Varicella (for Kindergarten and Grade 1)
- 1 dose of Varicella (for Grades 2-8)
- 1 Tetanus Diphtheria Pertussis (Tdap) for students entering Grade 7
Although a specific form is not required, it is recommended that Florida's Standardized School Entry Exam Form (DH3040) be used to document completion of a physical examination. The form must be completed by a health care provider licensed to perform physical examinations. For students transferring to a Florida School from another state, a comparable form would be acceptable, if completed within one year.
